This paper evaluates the costs and returns incurred by the use of chemical and biological crop products among households in five selected Compro communities in the derived, Southern Guinea, Northern Guinea, Sudan and Sahel Savanna agro-ecological zones (AEZs) in West Africa. Sixty households were randomly selected in each of the communities to give a total of 300 households. Data were collected on the characteristics of the chemical products, households' socio-economic variables such as age and education, as well as, on farm input and output quantities and prices in the 2009/2010 periods using a pre-tested questionnaire. Data were analyzed with descriptive statistics and budgetary techniques. The Results obtained show a male dominant, fairly literate farming household, with small landholdings (comprising mainly cereal and legume fields) that are predominantly inherited and located far away from the homestead. Inorganic fertilizers, organic manure, improved seeds and pesticides are known as commercial inputs/ products used on farmers' fields, while agrolizer, apron plus and boost extra are the emerging products. The average quantity of inputs applied varied across the zones. The total quantity of inorganic fertilizer applied on the fields was highest in the NGS (924 kg) and lowest in the Sudan (676 kg). However, fertilizer application per hectare by respondents was below recommended dosages across the zones. The emerging chemical inputs (Agrolizer, Boost Extra and Apron Plus) were used only in Compro communities in the derived savanna (DS) and southern guinea savanna (SGS) by a small number of households. The results obtained from budgetary analysis show that gross margin per hectare was highest in the SGS ($ 254) where the emerging inputs were used by 41.7% of the households and lowest in the Sahel ($ 76). Organic fertilizer was used only in small quantities in the AEZs. Total variable costs accounted for more than 30% of revenue generated, and labour and fertilizer accounted for the highest percentage of these costs. The study concludes that promoting the emerging chemical inputs through increased accessibility and farmers' training on their appropriate agronomic use would increase farmers' income generating potentials for sustainable crop production across the AEZs.

Hypertension is one of the most important risk factors for stroke and cardiovascular diseases (CVD) globally. Understanding risk factors for hypertension among individuals with matching characteristics with stroke patients may inform primordial/primary prevention of hypertension and stroke among them. This study identified the risk factors for hypertension among community-dwelling stroke-free population in Ghana and Nigeria. Data for 4267 community-dwelling stroke-free controls subjects in the Stroke Investigative Research and Education Network (SIREN) study in Nigeria and Ghana were used. Participants were comprehensively assessed for sociodemographic, lifestyle and metabolic factors using standard methods. Hypertension was defined as a previous diagnosis by a health professional or use of an anti-hypertensive drug or mean systolic blood pressure ≥ 140 mmHg and/or diastolic blood pressure ≥ 90 mmHg. Logistic regression analysis was used to estimate adjusted odds ratios (aOR) of hypertension and their 95% confidence intervals (CI) at p <.05. Overall, 56.7% of the participants were hypertensive with a higher proportion among respondents aged ≥60 years (53.0%). Factors including physical inactivity (aOR: 9.09; 95% CI: 4.03 to 20.53, p <.0001), diabetes (aOR: 2.70; CI: 1.91 to 3.82, p <.0001), being ≥60 years (aOR: 2.22; 95% CI: 1.78 to 2.77, p <.0001), and family history of CVD (aOR 2.02; CI: 1.59 to 2.56, p <.0001) were associated with increased aOR of hypertension. Lifestyle factors were associated with hypertension in the current population of community-dwelling stroke-free controls in west Africa. Community-oriented interventions to address sedentary lifestyles may benefit this population and reduce/prevent hypertension and stroke among them.

Background: Stroke risk can be quantified using risk factors whose effect sizes vary by geography and race. No stroke risk assessment tool exists to estimate aggregate stroke risk for indigenous African. Objectives: To develop Afrocentric risk-scoring models for stroke occurrence. Materials and Methods: We evaluated 3533 radiologically confirmed West African stroke cases paired 1:1 with age-, and sex-matched stroke-free controls in the SIREN study. The 7,066 subjects were randomly split into a training and testing set at the ratio of 85:15. Conditional logistic regression models were constructed by including 17 putative factors linked to stroke occurrence using the training set. Significant risk factors were assigned constant and standardized statistical weights based on regression coefficients (β) to develop an additive risk scoring system on a scale of 0–100%. Using the testing set, Receiver Operating Characteristics (ROC) curves were constructed to obtain a total score to serve as cut-off to discriminate between cases and controls. We calculated sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) at this cut-off. Results: For stroke occurrence, we identified 15 traditional vascular factors. Cohen's kappa for validity was maximal at a total risk score of 56% using both statistical weighting approaches to risk quantification and in both datasets. The risk score had a predictive accuracy of 76% (95%CI: 74–79%), sensitivity of 80.3%, specificity of 63.0%, PPV of 68.5% and NPV of 76.2% in the test dataset. For ischemic strokes, 12 risk factors had predictive accuracy of 78% (95%CI: 74–81%). For hemorrhagic strokes, 7 factors had a predictive accuracy of 79% (95%CI: 73–84%). Conclusions: The SIREN models quantify aggregate stroke risk in indigenous West Africans with good accuracy. Prospective studies are needed to validate this instrument for stroke prevention.

Background:High sensitivity C-reactive protein (hsCRP) and Plasminogen Activator Inhibitor-1 (PAI-1) are recognised independent novel risk factors for cardiovascular disease. Few studies have assessed cardiovascular risk factors in patients with hyperglycaemic emergencies (HE), despite it being a major cause of death in diabetics.
Objective:
The objective of this study was to determine cardiovascular risk indices in patients with hyperglycaemic emergencies and related these with outcome.
Methods:
This cross sectional study involved 45 patients that presented with HE and 45 age and sex matched diabetics without HE who served as controls. Historical features, physical findings and laboratory parameters including hsCRP and PAI-1 were compared between subjects and controls.
Results:
The mean values of serum hsCRP and PAI-1 were significantly higher in patients with HE compared to diabetic control. (49.52 ± 13.6 vs. 2.4 ± 1.35, 51.2 ± 28.7 vs. 33.2 ± 10.7 respectively). Traditional cardiovascular risk factors such as HbA1c, Atherogenic Index and microalbuminuria were also significantly higher in them. Mortality was associated with increasing age, higher values of waist circumference, pulse rate, respiratory rate, hsCRP, Atherogenic index and lower blood pressure and HDL values.
Conclusion:
Cardiovascular risk indices are higher in patients with HE.

Aims:Glucose tolerance abnormalities are frequently observed in patients with chronic liver disease (CLD). Insulin resistance (IR) has been suggested to be a major factor responsible for these abnormalities in CLD. However studies relating IR with severity of CLD are scarce in Nigeria. This study assessed insulin resistance and glucose tolerance abnormalities in CLD and their relationship with the severity of CLD in a tertiary hospital in South-West, Nigeria.
Methods:
This cross sectional study involved 100 subjects with CLD. Ethical clearance was obtained and informed consent was granted by participants. Participants were interviewed using a structured proforma; physical examination and relevant investigations were performed. Insulin resistance was measured using the homeostasis model assessment (HOMA-IR) Data was analysed using Statistical Package for Social Sciences version 20.0 and p value of <0.05 was considered significant.
Results:
Mean age of the study participants was 51.9 ± 11.9 years, and mean duration of CLD was 15.9 ± 5.8 months. Glucose tolerance abnormalities were present in 66 subjects (66%) and increased from 16.1% in Child Pugh's class A to 90.0% in class C. HOMA-IR positively correlated with age, body mass index, serum blood glucose, duration and severity of CLD. Increasing age, presence of hepatocellular carcinoma, Child Pugh's class B and class C were associated with glucose tolerance abnormalities.
Conclusion:
Glucose tolerance abnormalities and insulin resistance were highly prevalent among chronic liver disease subjects studied and seemed to parallel the severity of CLD, determined by the Child Pugh's score.

Vehicle plate number recognition plays an important role in traffic control and surveillance systems. A key stage in any vehicle plate number recognition system is to first locate the vehicle plate number. In this paper, we present a modified GrabCut algorithm for localizing vehicle plate numbers. In contrast with the traditional interactive GrabCut technique, a modified GrabCut algorithm was designed to identify and extract vehicle plate numbers in a completely automatic manner. Our approach extends the use of the traditional GrabCut algorithm with addition of a feature extraction method which uses geometric information to give accurate foreground extraction. Finally, to evaluate the performance of the proposed technique, the localization accuracy is tested with a dataset of 500 vehicle images with vehicle plates from different countries. An accuracy of 99.8% was achieved for the localization of vehicle plates. Comparative analysis is also reported.
